On November 18, 2021, the National Medical Commission (NMC) released the Foreign Medical Graduate Licensure (FMGL) Regulations, a 12-page Gazette notification that fundamentally changed the landscape for Indian students pursuing medical degrees outside India.
This regulation moved away from the previous system of simply verifying the university's recognition and introduced strict criteria regarding course duration, internship location, and local licensing. Any student who fails to meet even one of these criteria will be ineligible to sit for the National Exit Test (NExT) or practice medicine in India upon their return. At MatchToCollege, we maintain a live database of global medical curricula to cross-reference university syllabus structures against these specific 2021 mandates.
Before you commit to a foreign university, you must verify that the institution provides a minimum of 54 months of medical education and a mandatory 12-month internship conducted within the same hospital.
To see how your current NEET scores align with domestic options before looking abroad, you can use our NEET College Predictor to evaluate your standing. The FMGL regulations are not merely suggestions; they are legal requirements that have already led to the disqualification of certain pathway programs and 3-year BS+MD courses formerly popular in regions like the Philippines and parts of the Caribbean.
The 54+12 Mandate: Analyzing the Duration Requirements
The core of the NMC FMGL 2021 regulations is the strict timeline of education. According to Section 4(a)(i), the course of study leading to the foreign medical degree must have a minimum duration of fifty-four (54) months.
This is strictly defined as academic study time and does not include preparatory or foundation years. Furthermore, Section 4(a)(ii) mandates an internship of at least twelve (12) months in the same foreign medical institution where the degree was obtained. This 'same institution' clause is critical; students can no longer complete their clinical rotations or internships in different countries or back in India and expect it to count toward their degree completion for licensing purposes.
For students planning their 2026 intake, calculating these months accurately is vital. If a university claims a 4-year program (48 months), it is immediately non-compliant with Indian law.
Our team at MatchToCollege frequently reviews the transcript structures of universities listed in our MBBS Abroad directory to ensure they meet the 54-month academic threshold. FAQFrequently Asked Questions
Q: Can I do my internship in India to satisfy the FMGL 12-month rule?
No. The 2021 FMGL regulations specifically state that the 12-month internship must be completed in the same foreign medical institution where the 54-month course was completed. You will still be required to do a second 12-month internship (CRMI) in India after passing the NExT exam, but the first one must be done abroad.
Q: Is the 54-month duration inclusive of the internship?
No. The 54 months (4.5 years) are for the academic course alone. The 12-month internship is additional, making the total duration at least 5.5 years (66 months) of stay in the foreign country.
Q: Does the NMC recognize MBBS degrees from the Philippines?
Currently, the 4-year MD program in the Philippines is under scrutiny because the preceding BS course is not considered part of the medical degree by the NMC. Following the FMGL 2021 rules, students are advised to only choose programs that offer a continuous 54-month medical course post-12th standard.

Quick answer: For the PGP 2027–29 batch (CAT 2026), IIM Ahmedabad's minimum cutoff is 95 percentile overall and 85 in each section for General/EWS candidates. Reports of the revised policy say the cutoffs are unchanged from the previous cycle, which had already jumped from 80/70 the year before. Clearing the minimum only makes you eligible; it does not guarantee an interview call.
